Directions
  1. An Official Transcript is required from each institution attended. High School/GED, Post-Secondary Schools, CLEP/DANTES, Vocational/Technical Training, Military Training Documents (ex. DD214)
  2. Corporate Training Record/Certificates (some training may receive College Credit)
  3. To be official, the Transcript must be sent directly from the issuing institution to Student Records at Bellevue University. (Student copies are not official)
  4. Student must contact each institution for amount of transcript fee and address.
  5. Fill in the request form and send with transcript fee to the institution. This should be done as soon as possible.
